Adult Day Services Admission Process: 

1. Contact your Care Manager - Management Care Organization or IRIS
2. ​Once set up with a plan - Reach out to Portal Inc's Adult Day Services (ADS) manager - Lori Bares
3. ​Portal Inc will then begin the "Pre-Admission" process, providing necessary paperwork to gather essential information about the individual.​
4. The Adult Day Services Manager at Portal Inc will engage with the family and the appropriate Care Manager to review the admission application.
5. If there's an available slot and the individual seems to be a suitable match for the program, an on-site visit and tour will be arranged.
6. If the fit is confirmed, the full admission process will be coordinated with the family and the Management Care Organization or IRIS.
